HomeChurch Team Ministry Authorization & Release

To complete a comprehensive criminal background check, an address update will be conducted. The Fair Credit Reporting Act requires a release to be signed authorizing access to this information. This information allows us to determine the various legal jurisdictions from which data must be received to complete the screening process. Neither HomeChurch nor its agent, Checkr, will access financially oriented information and is prevented from doing so by FCRA, and Act of Congress.

I understand that I am entitled to a complete and accurate disclosure of the nature and scope of any comprehensive criminal background check of which I am subject upon my written request to Checkr if it is made within a reasonable time. I will also receive from HomeChurch a written summary of my rights if requested.

In connection with my application with HomeChurch, I authorize HomeChurch and/or Checkr, their agent, to solicit background information relative to my criminal record history. I understand HomeChurch may conduct inquiries into my background which may include criminal records, my driving history, personal references provided by me, and other public records pertaining to me.
